Philosophy of the
TBI Waiver
The dignity of
risk and right to fail are integral parts of the waiver’s
philosophy. The philosophy of the waiver supports the participant’s
right to choose where to live, who to live and socialize with, and
what goals and activities to pursue. Waiver services are provided
based on the participant’s unique strengths, needs, choices
and goals. The individual is the primary decision-maker and works in
cooperation with providers to develop a plan for services. This
process leads to personal empowerment, increased independence,
greater community inclusion, self-reliance and meaningful and
productive activities.
Who is eligible
and should apply?
Adults with a
diagnosis of traumatic brain injury (TBI).
NOTE: Individuals who experience deficits similar
to a traumatic brain injury as a result of anoxia, toxic poisoning,
stroke or other neurological conditions may also be eligible.
Be between the ages of 18 and 64 upon application to the waiver.
People that have a desire to be as independent as possible should
contact our team of professionals to discuss
eligibility and steps to
apply. If you or your loved one is currently in a nursing home or
other facility and desires to go home, this waiver offers services
to establish a safe discharge and ongoing support services.

Ably's TBI Services
Initial Service Coordination: Provides coordination in becoming a waiver participant and when needed coordinates a safe transition from inpatient.
Ongoing service Coordination: Assists client and family in assessing, accessing and coordinating all services needed to safely remain in the home.
Community
Integration Counseling
Independent Living Skills: Provides a comprehensive functional assessment with ongoing education/training in areas identified.
